Getting older brings new challenges. Our Case Management Program is here to help adults age 60 and older stay in their own homes and communities as they age.

If you or an older adult you know answers “Yes” to any of the following questions, you or they may be eligible for support services.

Has preparing meals become more difficult?

Have you had a hard time getting out to shop for groceries?

Could you use some help with housekeeping or chores?

We can send a case manager for a home visit to find out exactly what is needed for you to stay healthy and mange those activities that may have become more difficult with age.
